Purpose: If you or your department has found a data issue, the Office of Planning, Effectiveness, Analytics and Research will find the source of the data and notify the department responsible for it.

Please indicate the nature of the discrepancy found below as well as the purpose of the data and when it is needed. Also note whether the discrepant data is limited to a small number of instances (if so, please attach information regarding what instances) or if the issue is widespread.

Please include your contact information in case further clarification is required. Depending on the nature of the discrepancy and the number of cases affected, expect at least a two-week turnaround or longer.
